Objective To investigate the effect of edaravone dexcamphenol injection with concentrated solution combined with butylphthalide in the treatment of acute cerebral infarction. Methods This was a retrospective study. A total of 76 cases of acute cerebral infarction in Affiliated Hospital of Xuzhou Medical University from January 2020 to October 2022 were selected and were divided into a conventional group and a treatment group according to different treatment methods with 38 cases in each group. In the conventional group, there were 20 males and 18 females; the age ranged from 39 to 81 years old, with a mean of (59.36±9.25) years old. In the treatment group, there were 21 males and 17 females; the age ranged from 38 to 80 years old, with a mean of (58.69±9.96) years old. The conventional group was treated with butylphthalide, and the treatment group was treated with edaravone dexcamphenol injection with concentrated solution combined with butylphthalide. The efficacy and neurological function indexes [National Institutes of Health Stroke Scale (NIHSS) score], Barthel indexes (BI), and Ability of Daily Living Scale (ADL) scores before treatment and 2 weeks after treatment were compared between the two groups. The statistical methods used were independent sample t test, paired t test, χ2 test, and rank sum test. Results The total effective rate of the treatment group was higher than that of the conventional group [94.74% (36/38) vs. 78.95% (30/38)], with a statistically significant difference (χ2=4.145, P=0.042). There was no statistically significant difference in the NIHSS score between the two groups before treatment (t=0.194, P=0.847). After 2 weeks of treatment, the NIHSS scores of both groups were decreased (both P<0.05), and the NIHSS score of the treatment group was lower than that of the conventional group [(12.37±3.12) points vs. (19.36±2.96) points, t=10.019, P<0.001]. Before treatment, there were no statistically significant differences in the BI and ADL scores between the two groups (t=0.233, P=0.817; t=0.337, P=0.737). After 2 weeks of treatment, the BI and ADL scores in both groups were higher than those before treatment (all P<0.05), the BI score [(22.98±3.58) points vs. (18.25±3.01) points, t=6.234, P<0.001] and ADL score [(84.21±8.92) points vs. (77.69±7.69) points, t=3.413, P=0.001] in the treatment group were higher than those in the conventional group. Conclusion In the treatment of patients with acute cerebral infarction, edaravone dexcamphenol injection with concentrated solution combined with butylphthalide can effectively improve the therapeutic effect, neurological function, and daily living ability, which is worthy of popularization and application.

    International Medicine and Health Guidance News    2023, 29 (14): 1962-1966.   DOI: 10.3760/cma.j.issn.1007-1245.2023.14.009
    Abstract368)            Save

Objective To explore the factors causing the high false-positive rate in human immunodeficiency virus (HIV) detection by chemiluminescence microparticle immunoassay (CMIA). Methods The medical records of 295 patients positive in HIV detection by CMIA in First Hospital Affiliated to Sun Yat-sen University from January 2018 to December 2020 were retrospectively analyzed. The patients were (47.4±16.7) years old. There were 185 males who were (47.5±17.4) years old and 110 females who were (47.2±15.5) years old. According to the results of HIV confirmatory test, the patients were divided into a true positive group (101 cases) and a false positive group (194 cases). The correlations of false positive results with detection time, gender, age, and S/CO value were analyzed. The case data of 155 patients positive in HIV detection by CMIA were collected, and were divided into a true positive group (77 cases), a false positive group (78 cases), and a negative group (78 cases). The relations of false positive with the patients' diagnosis and other laboratory indicators were analyzed. SPSS 24.0 was used for the statistical analysis. The ratios were compared by χ2 test. Results The false positive rates in 2018, 2019, and 2020 were 47.6% (39/82), 67.3% (66/98), and 77.4% (89/115), respectively, indicating that the false positive rate of HIV screening by CMIA increased year by year. The false positive rate in the patients under 20 or over 70 years old was relatively higher (P<0.05). The false positive rate in the female patients was higher than that in the male patients [87.3% (96/110) vs. 53.0% (98/185)], with a statistical difference (P<0.05). The S/CO value of the most patients with false positives was low; the false positive rates of the patients with diseases of the circulatory system, digestive system, urinary system, and reproductive system were higher. Conclusion Age, gender, S/CO value, and complications of circulatory system and digestive system may cause a higher false positive rate.

    International Medicine and Health Guidance News    2023, 29 (11): 1565-1568.   DOI: 10.3760/cma.j.issn.1007-1245.2023.11.019
    Abstract340)            Save

Objective To observe the effect of alprazolam in radical operation of lung cancer and its effects on preoperative anxiety and postoperative stress response. Methods This was a prospective randomized controlled trial. A total of 125 patients with lung cancer who received radical resection in The First Affiliated Hospital of Henan University from October 2020 to December 2022 were selected as the subjects, and were divided into two groups by the lottery method. In the control group, there were 42 males and 20 females, aged (60.44±5.36) years. In the observation group, there were 40 males and 23 females, aged (61.25±5.41) years. The control group only received routine intervention, and the observation group received alprazolam tablets assisted surgical treatment before surgery. The preoperative psychological state, preoperative and postoperative stresses, postoperative recovery, and occurrence of adverse drug reactions were compared between the two groups. Independent sample t test, χ2 test, and Fisher exact probability method were used for statistical analysis.  Results The scores of preoperative Self-rating Anxiety Scale (SAS) and state anxiety and trait anxiety of the State-Trait Anxiety Inventory (STAI) in the observation group were lower than those in the control group [(41.22±5.76) points vs. (44.49±5.33) points, (40.36±5.33) points vs. (44.46±5.49) points, (41.35±5.46) points vs. (45.11±5.47) points], with statistically significant differences (t=3.293, 4.237, and 3.846; all P<0.05). Before surgery, there were no statistically significant differences in the stress indexes between the two groups (all P>0.05). After surgery, the levels of norepinephrine (NE), cortisol (Cor), and prolactin (PRL) in the observation group were lower than those in the control group [(4.32±1.06) mg/kg vs. (5.77±1.36) mg/kg, (376.45±50.61) nmol/L vs. (405.33±50.12) nmol/L, (14.23±5.16) mmol/L vs. (17.44±5.28) mmol/L], with statistically significant differences (t=6.655, 3.205, and 3.438; all P<0.05). After surgery, the Visual Analog Scale (VAS) score [(4.11±1.24) points vs. (5.63±1.27) points], intrathoracic drainage volume within 24 after surgery [(88.45±10.47) ml vs. (93.11±10.36) ml], extubation time [(3.61±1.22) d vs. (4.77±1.48) d], and length of hospital stay [(6.64±1.23) d vs. (7.88±1.46) d] in the observation group were all lower than those in the control group, with statistically significant differences (t=6.771, 2.501, 4.785, and 5.174; all P<0.05). The incidence of drug-related adverse reactions in the observation group was slightly higher than that in the control group [6.35% (4/63) vs. 4.84% (3/62)], but the difference was not statistically significant (P>0.05). Conclusion Alprazolam assisted radical resection of lung cancer can reduce the patients' preoperative anxiety level and postoperative stress response, which is conducive to the patients' postoperative rehabilitation and does not increase the incidence of adverse reactions.

    International Medicine and Health Guidance News    2023, 29 (23): 3525-3528.   DOI: 10.3760/cma.j.issn.1007-1245.2023.23.045
    Abstract328)            Save

Objective To study the epidemiology of hand-foot-and-mouth disease (HFMD) and the spectrum of genotypes in the other enteroviruses (EVs) [non- enterovirus 71 (EV71) and non- Coxsaekievirus group A 16 (CA16)] in Guangzhou and Foshan area during and after the COVID-19 pandemic from 2022 to 2023. Methods EV universal type, EV71, and CA16 were detected by real time fluorescent reverse transcription polymerase chain reaction (RT-PCR) respectively for suspected HFMD samples from 2022 to 2023. The positive specimens of non-EV71 and non-CA16 were amplified and sequenced based on 5′ UTR primers. The spectrum of genotypes was analyzed with BLAST in NCBI on the basis of 5'-UTR primers. Results A total of 362 suspected HFMD specimens were collected in 2022, 47 (12.98%) of which were positive by real time simultaneous three virus detections. Among which, the positive rates of EV71, CA16, and non-EV71 and non-CA16 were 0, 1.38% (5/362), and 11.60% (42/362), respectively. A total of 297 suspected HFMD specimens were collected in 2023, 100 (33.67%) of which were positive by simultaneous three virus detections. Among which, the positive rates of EV71, CA16, and non-EV71 and non-CA16 were 0, 2.36% (7/297), and 31.31% (93/297), respectively. Sequence analysis of 51 non-EV71 non-CA16 positive samples showed that there were 9 genotypes of non-EV71 non-CA16 EVs detected from 2022 to 2023, which were CA6, CA10, CA4, CA2, CA8, CB5 (Coxsaekievirus group B 5), CB2, CB3, and E30 (echovirus 30), among which, CA6 accounted for 27.45% (14/51), followed by CA10, accounting for 15.69% (8/51). Conclusions The EV infection rate after the COVID-19 pandemic in 2023 was higher than that in 2022 during the COVID-19 pandemic. The non-EV71 and non-CA16 EVs were the main causative agent of HFMD during 2022-2023. The sequence analysis showed that CA6 and CA10 were the most prevalent pathogens of non-EV71 and non-CA16 EVs, so, more attention should be paid to CA6 and CA10 of other type EVs.

Application of sacubitril/valsartan in patients with heart failure taking dialysis

Xi Zhefan, Yang Jia, Zou Haozhen, Dong Hua
    International Medicine and Health Guidance News    2023, 29 (18): 2522-2525.   DOI: 10.3760/cma.j.issn.1007-1245.2023.18.002
    Abstract205)            Save

Sacubitril/valsartan is an angiotensin receptor neprilysin inhibitor. It can inhibit the breakdown of endogenous natriuretic peptides and block angiotensin receptors, thereby reducing the activation of the renin-angiotensin-aldosterone system. It can also promote myocardial remodeling, inhibit myocardial fibrosis, increase the glomerular filtration rate, and delay the deterioration of renal function. Sacubitril/valsartan is beneficial to patients with chronic kidney disease by reducing cardiovascular risk and delaying the progression of chronic kidney disease. As a result, sacubitril/valsartan is one of the important drugs for heart failure, hypertension, and uremia. However, its long-term use may also cause some adverse reactions, especially kidney damage. There are few reports on the application of sacubitril/valsartan in patients with end-stage of renal disease and heart failure. The purpose of this study is to observe the application of sacubitril/valsartan in maintenance dialysis patients with heart failure, to investigate its safety, and to provide references for researches in relevant fields.

    Abstract149)            Save

With the rise of national sports, the occurrence of sports injuries increases year by year, and at the same time, with the improvement of people's economic life, more attention is paid to the rehabilitation after sports injuries. Most of the current sports injury rehabilitation treatments mainly focus on limb function rehabilitation, but the changes in the brain function after sports injuries have also begun to receive attention from the researchers. Functional near-infrared spectroscopy (fNIRS) can be better applied in the detection of brain function in patients with sports injuries due to its anti-interference and portability. This paper summarizes the research results of sports injury rehabilitation and fNIRS, discusses the current situation and research progress of sports injury rehabilitation, and analyzes fNIRS and its application value in the study of the neural control mechanism of injured limbs after sports injuries, which puts forward new ideas to further enhance the rehabilitation effect of sports injuries.

    International Medicine and Health Guidance News    2024, 30 (6): 881-889.   DOI: 10.3760/cma.j.issn.1007-1245.2024.06.001
    Abstract147)            Save

Objective To investigate the target and mechanism of radix trichosanthis in the treatment of lung cancer based on network pharmacology and molecular docking. Methods The search period was up to July 2022. The active ingredients of radix trichosanthis were selected using the TCMSP and TCMID databases combined with the PubChem database. The potential targets of active ingredients were obtained using the TCMSP, TCMID, and SwissTargetPrediction databases. The GeneCards, OMIM, TTD, and DrugBank databases were searched to collect the targets of lung cancer. The drugs were intersected with disease targets and a Venn diagram was drawn using the online web site Venny 2.1. The common target protein-protein interaction network was drawn using the String database. The "drug-active ingredient-disease-target" network diagram was constructed through Cytoscape 3.8.2 software. Gene ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis of common targets were performed using Metascape database. Molecular docking verification was performed using Autodock software. Results Through screening, 28 active ingredients and 201 action targets were obtained from radix trichosanthis. A total of 1 645 lung cancer targets were retrieved from the four major disease databases. There were 37 intersecting targets between the drug and disease. According to the Degree value, estrogen receptor 1 (ER1), signal transduction and transcriptional activator 3 (STAT3), and epidermal growth factor receptor (EGFR) were determined as the core targets. GO enrichment analysis found that radix trichosanthis might treat lung cancer by regulating gland development, cell proliferation, cell adhesion, receptors, enzymes, and activities of proteins. The KEGG pathway enrichment analysis revealed that radix trichosanthis might act on chemical oncogenic receptor activation, pathways in cancer, hypoxia-inducing factor 1 (HIF-1) signaling pathway, and microRNAs and proteoglycans in cancer. Molecular docking results showed that the binding activity of key small molecule compounds to the core target was significant. Conclusion Radix trichosanthis plays a role in the treatment of lung cancer through multi-target and multi-pathway mechanisms, laying a foundation for subsequent experiments and clinical applications.

    Abstract143)            Save

Objective To investigate the relationship between the expression of tumor-infiltrating lymphocytes (TILs) and clinicopathological characteristics, and the effect of TILs on prognosis in patients with human epidermal growth factor receptor 2 (HER2) positive breast cancer. Methods A total of 94 female patients with HER2 positive breast cancer treated at Tengzhou Central People's Hospital from October 2016 to October 2018 were collected, with a median age of 51 years (32-73 years). Among them, 49 patients took trastuzumab and chemotherapy, and 45 chemotherapy. The degree of TILs in the tumor area of each patient's HE section was evaluated. χ² test was used to analyze the category data. The Kaplan Meier survival curve and Cox proportional hazard regression model were used to analyze the prognostic factors. The relationship between the degree of TILs and the clinicopathological characteristics and the effect of TILs on the patients' recurrence free survival (RFS) were analyzed. Results Among the 94 patients, 61 had low infiltration, and 33 high infiltration. There were no significant correlations between the degree of TILs and clinical pathological parameters (all P>0.05). In the targeted therapy group, 32 cases had low infiltration, and 17 cases high infiltration. The RFS of the patients with high TILs was longer than that of the patients with with low TILs (P=0.036); TNM stage and the degree of TILs were independent factors for the patients' prognosis (P=0.003 and 0.029). In the general chemotherapy group, 29 cases had low infiltration, and 16 cases high infiltration. The RFS of the patients with high TILs was longer than that of the patients with with low TILs (P=0.049); TNM stage and TILs were independent factors related to the patients' prognosis (P=0.001 and 0.028). Conclusions TILs can be used as tumor markers in predicting the prognosis and therapeutic effect of patients with HER2 positive breast cancer, and may become new therapeutic targets for tumors.

Protein phosphatases are a group of proteins that widely exist in human cells and catalyze protein dephosphorylation. The phosphoprotein phosphatase catalytic subunits (PPPCs) superfamily is a group of protein phosphatases that primarily dephosphorylate serine/threonine residues. The protein phosphatase-1 (PP1) is involved in most eukaryotic phosphorylation reaction and participates in regulating a variety of biological activities. The catalytic subunit α of PP1 (PP1A) is the most important subtype of the PP1 catalytic subunit encoded by PPP1CA, which plays an important role in the survival and metastasis of a variety of tumors, but it also exhibits inhibitory effect in tumors, which makes it a possible new target for tumor treatment. Therefore, this article aims to review the key role of PP1A in a variety of tumors and evaluate its potential as a novel target for tumor therapy.

This paper aimed to summarize the clinical application research progress, feasibility analysis, and mechanism research contents of hyperbaric oxygen chamber therapy synchronous rehabilitation therapy, discuss the existing research results, analyze the feasibility, and determine the next research direction. Literatures on rehabilitation in hyperbaric oxygen chamber was retrieved from CNKI and PubMed databases from January 2006 to December 2022, and the included literatures were analyzed from the aspects of research contents and research methods (experimental design, rehabilitation therapy techniques, research mechanisms, and evaluation methods). According to the research contents of the included literatures, they were roughly divided into 5 different directions according to the function: sports rehabilitation, cognitive rehabilitation, respiratory rehabilitation, musculoskeletal rehabilitation, and rehabilitation nursing. The hyperbaric oxygen in-cabin rehabilitation techniques involved in this study included comprehensive rehabilitation exercise training, motor ideation therapy, occupational cognitive therapy, neuromuscular electrical stimulation, bionic electrical stimulation, right median nerve electrical stimulation, acupuncture, artificial nose, and traction therapy, among which in-cabin physical therapy, occupational therapy, and traditional rehabilitation were the most common. In recent 10 years, the number of such studies had been increasing, and the experimental designs were diverse, including randomized controlled trials and paired trials. The subjects of the studies were mainly common patients in the rehabilitation department, and the curative effect evaluation methods were mainly various rehabilitation evaluation scales. The main mechanisms involved were: ① significantly improving the hemorheological indicators; ② promoting the repair of neurovascular units; ③ enhancing the central nerve plasticity; ④ activating related protease enzymes, growth factors, and triphosadenine (ATP). At present, the quantity and quality of clinical application researches of hyperbaric oxygen chamber rehabilitation therapy need to be improved, the clinical research contents are relatively simple, and the research methods are still in the exploratory stage. Most of the results of these studies focus on the clinical efficacy of dysfunction in the early post-traumatic brain injury and sequelae stage, and it is urgent to reveal the exact mechanism of action of hyperbaric chamber rehabilitation therapy through these studies. The effect of rehabilitation therapy in hyperbaric oxygen chamber still needs to be verified by further clinical mechanism research.

The patients were (55.65±2.14) years old. All the patients received P16/Ki-67 double staining test, liquid based cytology (TCT) examination, and high-risk human papillomavirus (HR-HPV) detection. The biopsy results under colposcopy were used as the gold standard. The measurement data were analyzed by t test, and the enumeration data by χ2 test. The value of HR-HPV, TCT, and P16/Ki-67 double staining in the diagnosis of cervical lesions was analyzed by the receiver operating characteristic curve (ROC). Results Among the 100 patients with cervical lesions, 14 cases (14.00%) were found with inflammatory lesions by colposcopy, 26 cases (26.00%) cervical intraepithelial neoplasia (CIN) Ⅰ, 26 cases (26.00%) CIN Ⅱ, 28 cases (28.00%) CIN Ⅲ, and 6 cases (6.00%) cervical squamous cell carcinoma (SCC). The sensitivity, specificity, and accuracy of HR-HPV in the diagnosis of cervical lesions were 97.6%(84/86), 85.71(12/14), and 96.00%(96/100), respectively; the sensitivity, specificity, and accuracy of TCT in the diagnosis of cervical lesions were 88.37%(76/86), 78.57%(11/14), and 87.00%(87/100), respectively; the sensitivity, specificity, and accuracy of P16/Ki-67 double staining in the diagnosis of cervical lesions were 91.86%(79/86), 85.71%(12/14), and 91.00%(91/100), respectively; there were no statistical differences between HR-HPV, TCT, and P16/Ki-67 double staining (all P>0.05). The areas under the curves (AUC) of HR-HPV, TCT, and P16/Ki-67 double staining in the diagnosis of cervical lesions were 0.917, 0.835, and 0.888, respectively. There were no statistical differences in AUC between P16/Ki-67 double staining on one hand and TCT and HR-HPV examination on the other hand (both P>0.05). Conclusion P16/Ki-67 double staining in screening cervical cancer and precancerous lesions is as effective as TCT and HR-HPV. It has the advantages of high efficiency, objectivity, simplicity, and repeatability, and can be used as a new choice for screening cervical cancer and precancerous lesions.
