Correlation between traditional Chinese medicine syndromes of Wangbi disease and pro-inflammatory factors

尪痹的中医证型与促炎因子的相关性研究进展

Abstract:

Wangbi disease was first proposed by the modern physician Professor Jiao Shude. It is a further embodiment of rheumatoid arthritis in the field of traditional Chinese medicine after the "Bizheng", "Lijie", and "Hexifeng". From the earliest record of the disease in the Huang Di Nei Jing to the present, after more than two thousand years, Wangbi disease is still one of the diseases recognized by the medical community that it is difficult to completely cure. Major doctors constantly improve the syndrome types of this disease based on their own clinical experiences. It was not until 2018 that Guidelines for the Diagnosis and Treatment of Rheumatoid Arthritis had the most comprehensive eight types of syndromes. Among them, five kinds of empirical evidences are cold and wet bi syndrome, rheumatism syndrome, hot and humid bi resistance certificate, blood stasis syndrome, and sputum stasis syndrome, and three kinds of deficiency evidences are Qi Yin deficiency certificate, Qi and blood deficiency certificate, and liver and kidney deficiency certificate. From a macro perspective, traditional Chinese medicine has achieved remarkable results under the guidance of the principles of syndrome differentiation and treatment. Western medicine believes that the occurrence and development of this disease are closely related to pro-inflammatory factors from a microscopic view, and the expressions of many pro-inflammatory factors vary in the different types of traditional Chinese medicine syndromes. This article summarizes the relationship between traditional Chinese medicine syndromes and pro-inflammatory factors in the pathogenesis of Wangbi, discusses the role of traditional Chinese medicine in the treatment of Wangbi disease development on inflammatory factors, in order to have a guiding effect on traditional Chinese medicine syndrome differentiation and combined traditional Chinese medicine and western medicine, and further improve the patients' survival quality.

摘要: 尪痹最早由现代医家焦树德教授提出,是继“痹症”“历节”“鹤膝风”之后中医界对类风湿关节炎的进一步具体化表达。从最早对本病有记载的《黄帝内经》到现在,历经两千多年后尪痹仍然是医学界公认的难以彻底治愈的疾病之一。各大医家根据自身临床经验对本病的证型不断完善,直到2018年在《类风湿关节炎病证诊疗指南》才有了最为全面的8种证型,其中5种实证分别是寒湿痹阻证、风湿痹阻证、湿热痹阻证、瘀血阻络证、痰瘀痹阻证,3种虚证分别是气阴两虚证、气血两虚证、肝肾不足证。中医从宏观角度在辨证论治原则的指导下收效斐然。西医从微观角度认为本病的发生发展与促炎因子关系密切,其中多种促炎因子的表达随着中医证型的不同有所差异。本文就尫痹发病过程中的中医证型与促炎因子相关性作一综述,探讨中医在治疗尫痹发展中对炎症因子的作用,以期对中医辨证论治和中西医联合用药上有指导作用,进一步提高患者生存质量。
