眼外伤患者凝血功能与眼压的相关性研究

Correlation between coagulation function and intraocular pressure in patients with ocular trauma

摘要:

目的 探讨眼外伤患者凝血功能与眼压的相关性。方法 将济南市第二人民医院眼外科2020年1月至2022年7月接收的眼外伤患者84例作为研究对象,其中男49例,女35例,年龄(45.80±15.41)岁。其中眼压正常62例,高眼压22例。入院后次日晨取空腹静脉血检测血常规及凝血功能,采用Goldman压平眼压计检测眼压,其中高眼压标准为>21 mmHg(1 mmHg=0.133 kPa)。统计学方法采用χ2检验、t检验、Pearson相关性分析。结果 84例眼外伤眼压为11~42 mmHg,平均值为18.76 mmHg;高眼压患者的凝血酶原时间(PT)、凝血酶时间(TT)、血浆D-二聚体(D-D)高于眼压正常的眼外伤患者[(18.95±3.10)s比(15.82±2.94)、(25.79±4.24)s比(21.68±4.05)s、(0.65±0.12)mg/L比(0.42±0.09)mg/L],血浆纤维蛋白原(Fib)低于眼压正常患者[(1.92±0.55)g/L比(2.45±0.61)g/L],差异均有统计学意义(t=4.227、4.043、8.975、3.510,均P<0.05)。眼外伤患者的眼压与TT、活化部分凝血活酶时间(APTT)及D-D呈显著的正相关性,相关系数分别为0.377、0.291、0.444;眼压与Fib呈显著的负相关性,相关系数为-0.233。结论 高眼压和TT、APTT及D-D有正相关性,与Fib有负相关性,眼外伤患者控制眼压治疗时,需考虑凝血功能异常因素。

Abstract:

Objective To explore the correlation between coagulation function and intraocular pressure in patients with ocular trauma. Methods Eighty-four patients with ocular trauma admitted to Department of Ophthalmic Surgery, Second People's Hospital of Jinan City from January 2020 to July 2022 were set as the study objects. There were 49 males and 35 females, with an age of (45.80±15.41) years. There were 62 cases with normal intraocular pressure and 22 cases with high intraocular pressure. The next day after admission, their fasting venous blood was collected to test their blood routine and coagulation function. The Goldman pressure flattening intraocular pressure meter was used to test their intraocular pressures. If the pressure is >21 mmHg (1 mmHg=0.133 kPa), the intraocular pressure is high. χ2 test, t test, and Pearson correlation analysis were used. Results The 84 patients' intraocular pressure was 11 to 42 mmHg, with an average of 18.76 mmHg. The prothrombin time (PT), thrombin time (TT), and plasma D-dimer (D-D) in the patients with high intraocular pressure were higher than those in the patients with normal intraocular pressure [(18.95±3.10) s vs. (15.82±2.94), (25.79±4.24) s vs. (21.68±4.05) s, and (0.65±0.12) mg/L vs. (0.42±0.09) mg/L]; the fibrinogen (Fib) in the patients with high intraocular pressure was lower than that in the patients with normal intraocular pressure [(1.92±0.55) g/L vs. (2.45±0.61) g/L]; there were statistical differences (t=4.227, 4.043, 8.975, and 3.510; all P<0.05). The intraocular pressure was positively correlated with TT, activated partial thromboplastin time (APTT), and D-D (r=0.377, 0.291, and 0.444), and negatively with Fib (r=-0.233) in the patients. Conclusions The intraocular pressure is positively correlated with TT, activated partial thromboplastin time (APTT), and D-D, and negatively with Fib in patients with ocular trauma. Abnormal coagulation factors need to be considered when controlling intraocular pressure in patients with ocular trauma.
