Clinical effect of thumbtack needle therapy for opioid-induced constipation in patients with cancer pain

揿针疗法干预癌痛患者阿片类药物相关性便秘的效果观察

Abstract:

Objective To evaluate the effect of thumbtack needle therapy for opioid related constipation in patients with cancer pain. Methods Eighty cancer pain patients with opioid related constipation who were admitted to Department of Oncology, Guangdong Second Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were selected as the research objects. They were divided into an observation group and a control group according to whether they took thumbtack needle therapy for constipation, with 40 patients in each group. There were 26 males and 18 females in the control group; they were (61.05±8.82) years old. There were 26 males and 16 females in the observation group; they were (61.83±9.09) years old. The control group received routine care for opioid related constipation; in addition, the observation group took thumbtack needle therapy. The scores of the Opioid Related Constipation Assessment Scale and the severities of constipation were compared between the two groups. t test, analysis of variance, and χ2 test were applied. Results Before the intervention, there were no statistical differences between the two groups in terms of gender, age, education level, body mass index, nutrition screening score, and score of Visual Analogue Scale (VAS) (all P>0.05). The scores of Opioid Related Constipation Assessment Scale before the intervention and on day 7 and 14 of the thumbtack needle therapy in the observation group were lower than those in the control group [(35.55±9.26) vs. (32.80±6.37), (29.35±5.81) vs. (36.08±8.86), and (23.23±6.45) vs. (37.95±7.65)]. The scores of bowel movement patterns, outcomes, and drug impact dimensions in the observation group were lower than those in the control group, with statistical differences (all P<0.05). The severities of opioid related constipation and on day 7 and 14 of the thumbtack needle therapy in the observation group were lower than those in the control group, with statistical differences between the two groups (all P<0.05). Conclusion Thumbtack needle therapy has a significant effect on opioid related constipation, improving the symptoms of opioid related constipation in patients with cancer pain and reducing the severity of opioid related constipation.

摘要:

目的 评价揿针疗法在癌痛患者阿片类药物相关性便秘中的应用效果。方法 选取2020年1月至2023年1月在广东省第二中医院肿瘤科收治的阿片类药物相关性便秘的癌痛患者作为研究对象,按是否采用揿针疗法干预便秘分为观察组和对照组,每组40例。对照组中,男22例,女18例,年龄(61.05±8.82)岁,采取常规阿片类药物相关性便秘护理。观察组中,男26例,女16例,年龄(61.83±9.09)岁,在对照组的基础上采用掀针进行辅助治疗。比较干预前后两组患者阿片类药物相关性便秘评估量表的评分结果和便秘严重程度。采用t检验、方差分析、χ2检验进行统计分析。结果 干预前,两组患者性别、年龄、文化程度、体质量指数、营养筛查评分、疼痛视觉模拟评分(VAS)等比较,差异均无统计学意义(均P>0.05)。观察组患者干预前和使用揿针疗法第7天、第14天阿片类药物相关性便秘评估量表评分分别为(35.55±9.26)分、(29.35±5.81)分和(23.23±6.45)分,均低于对照组同时段评分[(32.80±6.37)分、(36.08±8.86)分、(37.95±7.65)分](均P<0.05);干预前和干预后第7天、第14天排便规律、排便结局、药物影响维度得分观察组均低于对照组(均P<0.05)。观察组患者干预后第7天、第14天阿片类药物相关性便秘严重程度均低于对照组,两组比较差异有统计学意义(P<0.05)。结论 揿针疗法用于阿片类药物相关性便秘效果显著,改善癌痛患者的阿片类药物相关性便秘症状,降低阿片类药物相关性便秘严重程度。
