Efficacy of three-dimensional vaginal ultrasound in diagnosis of scar pregnancy classification

三维阴道超声对疤痕妊娠分型的诊断效能分析

Abstract:

Objective To analyze the efficacy of three-dimensional vaginal ultrasound in the diagnosis of scar pregnancy (SP) classification. Methods Sixty patients with SP treated at Haizhu District Women and Children Health Care Hospital from July 2020 to July 2023 were selected as the study objects. They were (31.52±4.27) years old. All the patients were examined by three-dimensional vaginal ultrasound. The results of surgical pathologic diagnosis were set as the gold standard. The diagnostic efficacy of three-dimensional vaginal ultrasound in the diagnosis of SP classification was explored. χ2  test was used. Results SP was detected in 59 cases by three-dimensional vaginal ultrasound, and one case was missed, with a diagnostic accuracy of 98.33% (59/60) and a missed diagnosis rate of 1.67% (1/60). There was no statistical difference in the SP detection rate between three-dimensional vaginal ultrasound and surgical pathology (χ2=0.099, P>0.05). The coincidence rates of three-dimensional vaginal ultrasound in the diagnosis of the simple pregnancy sac type and uterine type were 100.00% (43/43) and 100.00% (11/11), and that of mixed mass type was 83.33% (5/6). There was no statistical difference in the detection rate of SP in different types between three-dimensional vaginal ultrasound and surgical pathology (χ2=0.067, P>0.05). The coincidence rates of type I and type Ⅱ were 100.00% (41/41) and 100.00% (10/10), and that of type Ⅲ was 88.89% (8/9). Conclusion Three-dimensional vaginal ultrasound has a high diagnostic accuracy in SP classification, a high diagnostic coincidence rate for different SP classification and diagnostic image features, and a high diagnostic efficacy, so it is recommended to promote in clinical use.

目的 分析三维阴道超声对疤痕妊娠(scar pregnancy,SP)分型的诊断效能。方法 选取海珠区妇幼保健院2020年7月至2023年7月收治的(31.52±4.27)岁的SP患者60例为研究对象。所有SP患者均接受三维阴道超声检查,以手术病理诊断结果为金标准,探究三维阴道超声对SP分型的诊断效能。采用χ2检验。结果 三维阴道超声检出SP 59例,1例漏诊,诊断准确率为98.33%(59/60),漏诊率为1.67%(1/60);三维阴道超声SP检出率与手术病理比较,差异无统计学意义(χ2=0.099,P>0.05)。三维阴道超声诊断单纯妊娠囊型与宫腔型诊断符合率分别为100.00%(43/43)、100.00%(11/11),混合包块型符合率为83.33%(5/6);三维阴道超声SP不同分型检出率与手术病理比较,差异无统计学意义(χ2=0.067,P>0.05)。三维阴道超声诊断I型、Ⅱ型符合率分别为100.00%(41/41)、100.00%、(10/10),Ⅲ型符合率为88.89%(8/9)。结论 三维阴道超声在SP分型中诊断准确率高,对不同SP分型以及诊断图像特征的诊断符合率高,诊断效能高,建议临床推广使用。
