Analysis of semen quality and sex hormone levels in 131 obese and infertile male patients

131例肥胖不育男性患者精液质量及性激素水平分析

Abstract: Objective To analyze the influence of obesity on semen quality and sex hormone levels in infertile males. Methods A total of 131 obese and infertile male patients treated in Reproductive Medicine Center, Hospital of Seventy-third Group, Land Army from January 2018 to May 2021 were recruited into the obesity group, and they were 22-47 years old. During the same period, 160 healthy males receiving physical examination at Hospital of Seventy-third Group, Land Army were recruited into the healthy control group, and they were 25-44 years old. The semen qualities and sex hormone levels were compared between these two groups. The independent-sample t test was used for the statistical analysis. Results The sperm density, motility, forward movement rate in the obesity group were significantly lower than those in the control group [(47.43±21.19)×106 /ml vs. (65.09±28.58)×106/ml, (50.76±13.63)% vs. (63.39±18.30)%, and (30.94±13.76)% vs. (43.52±17.12)%; all P<0.05]. There were no statistical differences in the semen volume and pH value between these two groups (both P>0.05). The levels of serum testosterone (T) and luteinizing hormone (LH) were significantly lower and the serum estradiol (E2) was significantly higher in the obesity group than in the control group [(3.82±1.20) μg/L vs. (5.78±1.85) μg/L, (4.23±1.37) mIU/ml vs. (5.93±1.16) mIU/ml, and (36.11±11.52) ng/L vs. (27.26±13.14) ng/L; all P<0.05]. There was no statistical difference in the FSH level between these two groups (P>0.05). Conclusion Obesity can affect semen quality and sex hormone levels, and induce male infertility.

摘要: 目的 探讨肥胖对不育男性精液质量和性激素水平的影响。方法 选择2018年1月至2021年5月在中国人民解放军陆军第七十三集团军医院生殖医学中心就诊的肥胖不育男性患者131例为肥胖组,年龄22~47岁。健康对照组为160例同期在中国人民解放军陆军第七十三集团军医院进行体检的正常健康男性,年龄25~44岁。对以上两组实验对象进行精液质量分析及性激素水平测定,统计学分析采用独立样本t检验。结果 肥胖组精子密度、精子活率、精子前向运动率分别为(47.43±21.19)×106 /ml、(50.76±13.63)%、(30.94±13.76)%,均明显低于健康对照组的(65.09±28.58)×106 /ml、(63.39±18.30)%、(43.52±17.12)%,差异有统计学意义(均P<0.05);两组精液量及pH值差异均无统计学意义(均P>0.05)。肥胖组血清睾酮(testosterone,T)、黄体生成素(luteinizing hormone,LH)水平分别为(3.82±1.20)μg/L、(4.23±1.37)mIU/ml,均低于对照组的(5.78±1.85)μg/L、(5.93±1.16)mIU/ml;肥胖组男性患者血清雌二醇(estradiol,E2)水平为(36.11±11.52)ng/L,明显高于对照组的(27.26±13.14)ng/L;两组差异均有统计学意义(均P<0.05)。两组卵泡刺激素(follicle stimulating hormone,FSH)水平差异无统计学意义(P>0.05)。结论 肥胖会影响男性精液质量及性激素水平,从而引起男性不育。
