苯磺酸瑞马唑仑联合舒芬太尼在无痛胃镜检查中的应用

Application of remazolam benzenesulfonate and sufentanil in painless gastroscopy

摘要:

目的 探讨苯磺酸瑞马唑仑联合舒芬太尼在无痛胃镜检查中的应用效果。方法 选取2023年6月至12月于陕西省荣誉军人康复医院接受无痛胃镜检查的120例患者进行随机对照试验,采用随机数字表法将其分为观察组和对照组,各60例。观察组男33例,女27例,年龄(45.62±14.85)岁。对照组男36例,女24例,年龄(46.15±11.08)岁。对照组采用舒芬太尼联合丙泊酚麻醉,观察组采用苯磺酸瑞马唑仑联合舒芬太尼麻醉。比较两组围手术期情况、不同时间点平均动脉压(mean arterial pressure,MAP)、心率(heart rate,HR)、简易精神状态检查量表(Mini-mental State Examination,MMSE)评分及不良反应发生率。采用t检验和χ2检验进行统计分析。结果 观察组麻醉起效时间、苏醒时间、复苏室停留时间分别为(1.56±0.41)min、(4.69±0.85)min、(28.77±2.64)min,均短于对照组的(1.99±0.54)min、(5.77±1.12)min、(31.06±3.83)min,差异均有统计学意义(t=4.913、5.950、3.813,均P<0.05)。观察组检查开始时、检查结束时MAP、HR分别为(85.94±7.21)mmHg (1 mmHg=0.133kPa)、(91.25±6.83)mmHg、(70.11±6.84)次/min、(78.13±6.18)次/min,均高于对照组的(81.66±6.31)mmHg、(85.61±6.95)mmHg、(65.47±6.70)次/min、(74.58±5.69)次/min,差异均有统计学意义(t=3.460、4.483、3.754、3.273,均P<0.05)。观察组苏醒后10 min、20 min时MMSE评分分别为(25.18±2.11)分、(27.18±1.63)分,均高于对照组的(23.57±1.95)分、(26.06±1.77)分,差异均有统计学意义(t=4.341、3.605,均P<0.05)。结论 在无痛胃镜检查患者中,应用苯磺酸瑞马唑仑联合舒芬太尼可维持术中血流动力学稳定,对患者术后认知功能影响更小。

Abstract:

Objective To investigate the effect of remazolam benzenesulfonate and sufentanil in painless gastroscopy. Methods A total of 120 patients who received painless gastroscopy in Shaanxi Honorary Soldiers Rehabilitation Hospital from June to December 2023 were selected, and were divided into an observation group and a control group by the random number table method, with 60 cases in each group. There were 33 males and 27 females in the observation group who were (45.62±14.85) years old. There were 36 males and 24 females in the control group who were (46.15±11.08) years old. The control group were anesthetised with sufentanil and propofol, and the observation group with remazolam benzenesulfonate and sufentanil. The perioperative conditions, the mean arterial pressures (MAP), heart rates (HR), and scores of Mini-mental State Examination (MMSE) at different time points, and the incidence rates of adverse reactions were compared between the two groups. t and χ2 tests were used for the statistical analysis. Results The onset time of anesthesia, recovery time, and time staying in resuscitation room in the observation group were shorter than those in the control group [(1.56±0.41) min vs. (1.99±0.54) min, (4.69±0.85) min vs. (5.77±1.12) min, and (28.77±2.64) min vs. (31.06±3.83) min], with statistical differences (t=4.913, 5.950, and 3.813; all P<0.05). The MAP's and HR's at the beginning and end of the examination in the observation group were higher than those in the control group [(85.94±7.21) mmHg (1 mmHg=0.133kPa) vs. (81.66±6.31) mmHg, (91.25±6.83) mmHg vs. (85.61±6.95) mmHg, (70.11±6.84) times/min vs. (65.47±6.70) times/min, and (78.13±6.18) times/min vs. (74.58±5.69) times/min], with statistical differences (t=3.460, 4.483, 3.754, and 3.273; all P<0.05). The scores of MMSE 10 and 20 min after recovery in the observation group were higher than those in the control group (25.18±2.11 vs. 23.57±1.95 and 27.18±1.63 vs. 26.06±1.77), with statistical differences (t=4.341 and 3.605; both P<0.05). Conclusion The application of remazolam benzenesulfonate and sufentanil in patients taking painless gastroscopy can maintain the stability of intraoperative hemodynamics and has less effect on postoperative cognitive function.
