低温等离子射频消融术与CO2激光微创手术治疗喉癌患者对VHI及预后的影响

摘要:

目的 探讨低温等离子射频消融术与CO2激光微创手术治疗喉癌患者对术后嗓音障碍指数(voice handicap indexVHI)及预后的影响。方法 选取20176月至20216月威海市中心医院收治的86例喉癌患者,分为研究组(采用低温等离子射频消融术治疗)和对照组(采用CO2激光微创手术治疗)。对照组男29例,女14例,年龄438059.78±9.47)岁;研究组男27例,女16例,年龄397958.85±8.99)岁。对比两组患者术中出血量、手术时间、术后VHI、术后1个月黏膜恢复情况以及术后复发率。采用t检验、χ2检验。结果 与对照组相比,研究组术中出血量少(t=8.205P<0.001),手术时间较短(t=5.839P<0.001)。术后1个月两组患者嗓音功能VHI评分均降低,且研究组低于对照组[(4.25±0.57)分比(6.85±1.04)分](t=14.376P<0.001)。两组患者术后复发率对比,研究组低于对照组[4.65%2/43)比20.93%9/43)],差异有统计学意义(χ2=5.108P=0.024)。研究组术后1个月黏膜恢复情况优于对照组(χ2=4.074P=0.044)。结论 低温等离子射频消融术治疗喉癌患者手术用时短,出血量少,改善患者的嗓音,改善患者预后,值得推广。

Abstract:

Objective To investigate the effects of low-temperature plasma radiofrequency ablation and CO2 laser minimally invasive surgery on postoperative voice handicap index (VHI) and prognosis in patients with laryngeal cancer. Methods A total of 86 cases of laryngeal cancer admitted to Weihai Central Hospital from June 2017 to June 2021 were retrospectively selected and were divided into a study group treated by low-temperature plasma radiofrequency ablation and a control group treated by CO2 laser minimally invasive surgery. In the control group, there were 29 males and 14 females, aged 43-80 (59.78±9.47) years; in the study group, there were 27 males and 16 females, aged 39-79 (58.85±8.99) years. The intraoperative blood loss, operative time, postoperative VHI score, mucosal recovery 1 month after surgery, and postoperative recurrence rate were compared between the two groups. t test and χ2 test were used. Results Compared with those in the control group, the study group had less intraoperative blood loss (t=8.205, P<0.001) and shorter operation time (t=5.839, P<0.001). The VHI scores of voice function in both groups were decreased 1 month after surgery, and the VHI score in the study group was lower than that in the control group [(4.25±0.57) points vs. (6.85±1.04) points] (t=14.376, P<0.001). The postoperative recurrence rate in the study group was lower than that in the control group [4.65% (2/43) vs. 20.93% (9/43)], with a statistically significant difference (χ2=5.108, P=0.024). The mucosal recovery of the study group was better than that of the control group 1 month after surgery (χ2=4.074, P=0.044). Conclusion Low temperature plasma radiofrequency ablation in the treatment of laryngeal cancer has short operation time and less blood loss, and can improve the patients' voice and prognosis, which is worth promoting.
