广州地区公职人员幽门螺杆菌感染情况及相关因素分析

Analysis of Helicobacter pylori infection and related factors among public officials in Guangzhou

摘要: 目的 分析探讨广州地区公职人员幽门螺杆菌(Hp)感染情况及相关影响因素,为 Hp 的科学防治提供决策依据。方法 收集 2020年 1月至 12月期间在广州市干部健康管理中心健康体检并进行尿素13C 呼气试验检测 Hp 感染的 6 436 例广州地区公职人员体检资料,其中男 3 535 例、女 2 901例,年龄(45.33±11.38)岁,进行回顾性分析,进行χ2 检验和二分类logistic回归分析,以了解广州地区公职人员Hp感染情况及Hp感染与性别、年龄、体质量指数(BMI)、血压、血糖、血脂、尿素、肌酐、 尿酸等因素之间的关系。结果 在6 436例受试者中Hp的总体检出率为30.53%(1 965/6 436),其中 男性为31.40%(1 110/3 535),女性为29.47%(855/2 901)。40~49岁年龄组男、女性Hp检出率分别为 32.27%(384/1 190)、27.06%(204/754),差异有统计学意义(P=0.015),其他年龄组男、女性 Hp检出率差异无统计学意义(P>0.05)。Hp阳性组的男女BMI分别为(24.86±2.94)kg/m2 、(22.42±2.81)kg/m2 ,高 于 Hp 阴性组(24.65±2.82)kg/m2 、(22.19±2.87)kg/m2 ;男性 Hp 阳性组的舒张压、三酰甘油分别为 (74.75±10.20)mmHg(1 mmHg=0.133 kPa)、(1.74±1.62)mmol/L,均高于 Hp阴性组(74.02±9.89)mmHg、 (1.60±1.58)mmol/L;女 性 Hp 阳 性 组 尿 酸 水 平 为(326.22±76.27)μmol/L,高 于 Hp 阴 性 组 (318.54±71.84)μmol/L,差异均有统计学意义(均 P<0.05)。多因素 logistic回归分析结果显示研究人 群Hp感染与性别、年龄、BMI、血压、血糖、血脂、尿酸无相关性(均P>0.05),仅影响尿素(OR=0.920,P< 0.01)和肌酐水平(OR=1.007,P<0.05)。结论 广州地区公职人员 Hp感染率无性别、年龄差异,感染 Hp人群代谢性指标无显著变化,但影响尿素水平和肌酐水平。

Abstract: Objective To analyze the Helicobacter pylori (Hp) infection and related influencing factors among public officials in Guangzhou, so as to provide decision-making basis for the scientific prevention and treatment of Hp. Methods The physical examination data of 6 436 public officials in Guangzhou who underwent physical examination and 13C urea breath test in Guangzhou Cadre Health Management Center from January to December 2020 were collected and analyzed retrospectively, including 3 535 males and 2 901 females, aged (45.33±11.38) years. The situation of Hp infection and the relationships between Hp infection and gender, age, body mass index (BMI), blood pressure, blood glucose, blood lipid, urea, creatinine, uric acid, and other factors were analyzed by Chi square test and binary logistic regression analysis. Results The infection rate of Hp was 30.53% (1 965/6 436) among 6 436 subjects, of which the male infection rate was 31.40% (1 110/3 535), and the female infection rate was 29.47% (855/2 901). The positive rates of Hp in males and females were 32.27% (384/1 190) and 27.06% (204/754) in the 40-49 years old group, respectively, with a statistically significant difference (P=0.015); there was no statistically significant difference in the positive rate of Hp between males and females in the other age groups (P>0.05). The male and female BMI of the Hp positive group were (24.86±2.94) kg/m2 and (22.42±2.81) kg/m2 , respectively, which were higher than those of the Hp negative group [(24.65±2.82) kg/m2 and (22.19±2.87) kg/m2 ]; the diastolic blood pressure and triglyceride in the male Hp positive group were (74.75±10.20) mmHg (1 mmHg=0.133 kPa) and (1.74±1.62) mmol/L, respectively, which were higher than those in the Hp negative group [(74.02±9.89) mmHg and (1.60±1.58) mmol/L]; the level of uric acid in the female Hp positive group was (326.22±76.27) μmol/L, which was higher than that in the Hp negative group [(318.54±71.84) μmol/L], with statistically significant differences (all P< 0.05). Multivariate logistic regression analysis showed that Hp infection had no correlation with gender, age, BMI, blood pressure, blood glucose, blood lipid, or uric acid (all P>0.05), but blood urea (OR=0.920, P<0.01) and creatinine (OR=1.007, P<0.05) were affected. Conclusions There are no significant gender or age differences in the infection rate of Hp among public officials in Guangzhou, and there are no significant differences in the metabolic indexes among Hp infected population, but the blood urea level and creatinine level are affected.
