Influences of health education based on trans-theoretical model guidance on self-management behaviors and quality of life in patients with pulmonary tuberculosis

基于跨理论模型指导的健康教育对肺结核患者自我管理行为及生活质量的影响

Abstract:

Objective To investigate the effect of health education based on trans-theoretical model (TTM) guidance on pulmonary tuberculosis (TB) patients. Methods It was a prospective randomized controlled trial. Using the random number table method, 80 cases of TB patients admitted to Nanyang Central Hospital from January 2020 to January 2022 were divided into two groups, with 40 cases in each group. In the observation group, there were 23 males and 17 females, with an age of (45.62±4.13) years old and a course of disease of (11.23±2.67) months. In the control group, there were 21 males and 19 females, with an age of (45.47±4.26) years old and a course of disease of (11.41±2.85) months. The observation group received health education based on TTM guidance, and the control group received routine health education. The disease cognition, self-management behaviors, and qualities of life in the two groups before and after intervention were compared, and the satisfaction degrees of nursing in the two groups were investigated. χ2 test was used for the count data and t test for the measurement data. Results After intervention, the disease cognition scores in both groups were higher than those before intervention, and the score in the observation group was higher than that in the control group [(84.31±6.32) points vs. (76.35±7.62) points], with statistically significant differences (all P<0.001). After intervention, the scores of Chronic Disease Self-Management Study Measures (CDSMS) in all dimensions in both groups were higher than those before intervention, and the scores in the observation group was higher than those in the control group (all P<0.05). After intervention, the scores of Generic Quality of Life Inventory 74 (GQOLI-74) in all dimensions in both groups were higher than those before intervention, and the scores in the observation group was higher than those in the control group (all P<0.05). The nursing satisfaction rate of the observation group was higher than that of the control group [95.00% (38/40) vs. 80.00% (32/40)], with a statistically significant difference between the two groups (χ2=4.114, P=0.043). Conclusion The use of health education based on TTM guidance in TB patients can improve the disease cognition and self-management behaviors, improve the patients' nursing satisfaction, and improve the quality of life.

目的 探讨肺结核患者采用基于跨理论模型(TTM)指导的健康教育的效果。方法 本文为前瞻性随机对照试验。选取20201月至20221月南阳市中心医院收治的80例肺结核患者,采用随机数字表法将其分为两组,各40例。观察组中男23例、女17例,年龄(45.62±4.13)岁,病程(11.23±2.67)个月,采用基于TTM指导的健康教育;对照组中男21例、女19例,年龄(45.47±4.26)岁,病程(11.41±2.85)个月,采用常规健康教育。比较两组干预前后的疾病认知、自我管理行为及生活质量,调查两组护理满意度。统计学方法采用t检验、χ2检验。结果 干预后两组疾病认知评分均高于干预前,且观察组高于对照组[(84.31±6.32)分比(76.35±7.62)分],差异均有统计学意义(均P<0.001);干预后两组慢性病自我管理研究测量量表(CDSMS)各维度评分均高于干预前,且观察组均高于对照组(均P<0.05);干预后两组生活质量综合评定问卷(GQOLI-74)各维度评分均高于干预前,且观察组均高于对照组(均P<0.05);观察组护理总满意度高于对照组[95.00%38/40)比80.00%32/40)],两组比较差异有统计学意义(χ2=4.114P=0.043)。结论 肺结核患者采用基于TTM指导的健康教育,可提高疾病认知及自我管理行为,提高患者的护理满意度,改善生活质量。
