Effect of vascular access group nursing on mood disorders in patients taking hemodialysis

血管通路小组护理对血液透析患者心境障碍的影响

Abstract:

Objective To explore the influence of vascular access group nursing on mood disorders in patients taking hemodialysis. Methods Sixty outpatients taking hemodialysis at Lianyungang Traditional Chinese Medicine Hospital from September 2021 to September 2022  were selected as the study objects. They were divided into a control group and an observation group by the random number grouping method, with 30 cases of each groups. There were 19 females and 11 females in the control group; they were 31-65 (50.16±4.87) years old. There were 18 males and 12 females in the observation group; they were 30-67 (50.49±4.28) years old. The control group took routine nursing, and the observation group vascular access group nursing. The scores of Self-Perceived Burden (SPB), Medical Coping Method Questionnaire (MCMQ), and Self-Management Behavior Scale were compared between these two groups. t test was applied. Results After the intervention, the score of SPB in the observation group was lower than that in the control group [(17.49±3.20) vs. (19.64±3.73)], with a statistical difference (t=2.396, P<0.05). After the intervention, the score of facing of MCMQ in the observation group was higher than that in the control group [(18.20±3.30) vs. (16.04±3.10)], and the scores of avoiding and surrender of MCMQ were lower than those in the control group [(13.67±2.12) vs. (14.79±2.20) and (6.10±1.02) vs. (6.94±1.09)], with statistical differences (t=2.613, 2.008, and 3.082; all P<0.05). After the intervention, the scores of partnership, self-care, problem solving, and emotional coping of self-management behaviors in the observation group were higher than those in the control group [(14.31±0.55) vs. (13.94±0.77), (17.71±0.45) vs. (17.25±0.60), (19.31±4.34) vs. (16.67±3.97), and (10.23±0.88) vs. (9.56±0.73)], with statistical differences (t=2.142, 3.359, 2.458, and 3.209; all P<0.05). Conclusion Vascular access group nursing for outpatients undergoing hemodialysis can improve their self-management burden and coping style, self-management behaviors, and prognosis and reduce their psychological barriers when facing diseases, so it is worth being learnt from and adopted by various departments.

目的 探究血管通路小组护理对血液透析患者心境障碍的影响。方法 前瞻性研究连云港市中医院2021年9月至2022年9月抽取的60例门诊血液透析患者,依照随机数字分组法分为两组,各30例。对照组男19例、女11例,年龄31~65(50.16±4.87)岁,采取常规护理;观察组男18例、女12例,年龄30~67(50.49±4.28)岁,应用血管通路小组护理。对比两组患者护理前后自我感受负担评分(SPB)、医学应对方式问卷(MCMQ)及自我管理行为量表评分。统计学方法采用t检验。结果 干预后,观察组患者SPB评分低于对照组[(17.49±3.20)分比(19.64±3.73)分],差异有统计学意义(t=2.396,P<0.05);干预后,观察组患者MCMQ评分中面对高于对照组[(18.20±3.30)分比(16.04±3.10)分],回避和屈服低于对照组[(13.67±2.12)分比(14.79±2.20)分、(6.10±1.02)分比(6.94±1.09)分],差异均有统计学意义(t=2.613、2.008、3.082,均P<0.05)。干预后,观察组血液透析患者自我管理行为评分(伙伴关系、执行自我护理、问题解决、情绪处理)均高于对照组[(14.31±0.55)分比(13.94±0.77)分、(17.71±0.45)分比(17.25±0.60)分、(19.31±4.34)分比(16.67±3.97)分、(10.23±0.88)分比(9.56±0.73)分],差异均有统计学意义(t=2.142、3.359、2.458、3.209,均P<0.05)。结论 针对门诊行血液透析的患者采取血管通路小组护理,可改善患者的自我管理负担、应对方式,降低患者面对疾病时内心的心境障碍,进而提高患者的自我管理行为方式,利于预后,值得各个科室借鉴采纳。
