Four cases of systemic lupus erythematosus complicated with lymphoma and literature review

系统性红斑狼疮合并淋巴瘤4例并文献复习

Abstract: This paper retrospectively analyzed the clinical data of 4 patients with systemic lupus erythematosus (SLE) and lymphoma, and the related literatures were reviewed to investigate whether there was a relationship between SLE and lymphoma, to provide experiences for the clinical practice in the future, and to avoid missed diagnosis and misdiagnosis. All the 4 patients were female, and were 47-61 years old. Among the 4 patients, 3 developed lymphoma with a SLE duration of 7-10 years, while lymphoma was detected in 1 patients just 3 months after being diagnosed as SLE. In terms of the outcomes of the 4 patients, 3 responded well to chemotherapy, but 1 abandoned therapy due to being in a critical condition and died finally. The clinical characteristics are quite similar between SLE and lymphoma, and they are hard to identify. This finding may suggest that more attention should be paid to the incidence of lymphoma when SLE patients suffer from fever, lymphadenopathy, or blood cell reduction, and taking effective treatment on this kind of patients early may contribute better life quality.

摘要: 本文通过回顾性分析4例系统性红斑狼疮(systemic lupus erythematosus,SLE)合并淋巴瘤患者的临床资料并复习相关文献,探讨两者的关系,为今后临床实践提供经验,避免漏诊和误诊。4例患者均为女性,年龄47~61岁,3例在确诊淋巴瘤时均有长病程的SLE病史(7~10年),1例诊断SLE 3个月后确诊淋巴瘤。其中3例经化疗后病情稳定,1例因病情危重,化疗风险高,患者及家属放弃治疗后死亡。SLE合并淋巴瘤时因两者临床表现相似,鉴别困难,当患者出现发热、淋巴结肿大、血细胞减少时,应当引起风湿免疫科医生的重视,警惕淋巴瘤的可能,早期干预,积极治疗,改善患者的生活质量及预后。
