Effect observation of arthroscopy in the treatment of knee dislocation complicated with ligament injury

关节镜在膝关节脱位并发韧带损伤治疗中的应用效果观察

Abstract:

Objective To analyze the effect of arthroscopy in the treatment of knee dislocation complicated with ligament injury. Methods It was a randomized controlled trial. The clinical data of 56 patients with knee dislocation complicated with ligament injury admitted to Department of Orthopedics, Linyi Traditional Chinese Medical Hospital from January 2020 to July 2022 were analyzed, and were divided into two groups by the random number table method. The 28 patients in the control group received routine surgery, including 19 males and 9 females, aged 22-63 (42.52±8.52) years. The 28 patients in the observation group received arthroscopic surgery, including 21 males and 7 females, aged 20-62 (43.51±8.39) years. The Visual Analogue Scale (VAS) score, joint extension angle and flexion angle, International Knee Documentation Committee Knee Function Form (IKDC) score, Lysholm score, and complication rate were compared and analyzed between the two groups. t test and χ2 test were used for statistical analysis. Results After surgery, the VAS score of the observation group was significantly lower than that of the control group [(3.09±0.73) points vs. (3.52±0.69) points], with a statistically significant difference (t=2.265, P=0.028); the flexion angle of the observation group was significantly higher than that of the control group [(116.49±5.62)° vs. (113.25±5.52)°], with a statistically significant difference (t=2.174, P=0.034); the IKDC and Lysholm scores in the observation group [(88.54±9.75) points and (79.56±9.65) points] were significantly higher than those in the control group [(83.25±9.25) points and (73.52±8.52) points], with statistically significant differences (t=2.083 and 2.483, both P<0.05). The complication rate of the observation group was lower than that of the control group [7.14% (2/28) vs. 28.57% (8/28)], with a statistically significant difference (χ2=4.383, P=0.036). Conclusion The application of arthroscopy in the treatment of patients with knee dislocation complicated with ligament injury can effectively reduce the patients' postoperative pain, improve the level of knee joint functional recovery, at the same time, the incidence of complications is low, and has a high application value.

摘要:

目的 分析关节镜在膝关节脱位并发韧带损伤治疗中的应用效果。方法 本研究为随机对照试验。分析临沂市中医医院骨科2020年1月至2022年7月期间收治的膝关节脱位并发韧带损伤患者56例临床资料,患者按随机数字表法分为两组。对照组28例接受常规手术治疗,其中男19例、女9例,年龄22~63(42.52±8.52)岁;观察组28例应用关节镜手术治疗,其中男21例、女7例,年龄20~62(43.51±8.39)岁。对比分析两组患者视觉模拟评分法(Visual Analogue Scale,VAS)评分、关节伸直角度、屈曲角度、国际膝关节文献委员会膝关节功能评估量表(International Knee Documentation Committee Knee Function Form,IKDC)评分、Lysholm评分以及并发症发生率。统计学方法采用t检验、χ2检验。结果 术后,观察组患者VAS评分(3.09±0.73)分,显著低于对照组(3.52±0.69)分,差异有统计学意义(t=2.265,P=0.028);观察组屈曲角度(116.49±5.62)°,显著高于对照组(113.25±5.52)°,差异有统计学意义(t=2.174,P=0.034);观察组IKDC、Lysholm评分(88.54±9.75)分、(79.56±9.65)分,均显著高于对照组(83.25±9.25)分、(73.52±8.52)分,差异均有统计学意义(t=2.083、2.483,均P<0.05)。观察组患者并发症发生率7.14%(2/28)显著低于对照组28.57%(8/28),差异有统计学意义(χ2=4.383,P=0.036)。结论 应用关节镜为膝关节脱位并发韧带损伤患者进行治疗可有效降低患者术后疼痛感受,提高患者膝关节功能恢复水平,同时并发症发生率较低,具有较高的推广应用价值。
